ESAH Supervisor Job Summary
The New York City Housing Authority’s (NYCHA) Family Partnerships Elderly Safe at Home Grant-funded Program is seeking candidates to work as a supervisor to oversee the ESAH Bronx case assistants who provide a wide range of supportive services to elderly and disabled residents. Travel required.
Job Duties
· Oversee the daily operations of the ESAH sites in the Bronx.
· Conduct home visits and wellness checks to elderly and disabled residents.
· Conduct regular performance evaluations and provide ongoing training and support to ESAH staff.
· Ensure ESAH case assistants link elderly residents and those who are frail or at risk to supportive services.
· Review and sign off on case management activity in each case.
· Monitor case files and documentation for accuracy, completeness, and compliance.
· Complete monthly reports
· Manage the Floor Captain/Buddy system program to monitor residents’ well-being.
· Coordinate monthly workshops and meetings.
· Manage the monthly workshops budget and receipts for all sites.
· Ensure activation of outreach to ESAH residents.
· For the health and well-being of residents during heat, fire, and snow emergencies.
· Ensure accurate and timely case logs and reports.
· Make sure that ESAH case assistants work no more than 35 hours per week.
· Monitor staff’s time and attendance.
· Establish and sustain formal partnerships with relevant social service providers.
· Attend supervisory and staff meetings, trainings, and community meetings/events as requested by the supervisor.
· Build and maintain communication with the Property Management staff and Resident Leaders.
Minimum Qualification Requirements
· Bachelor’s degree from an accredited college with at least 5 years of experience in case management, including home visits. · Associate’s degree with at least 10 years’ experience in case management, including home visits.
· Supervisory experience.
Preferred Skills
· Bachelor’s degree with at least five (5) years of case management experience in a social service setting.
· Excellent written and verbal communication skills.
· Experience engaging community-based agencies, networking, and creating partnerships.
· Ability to communicate in both English and Spanish.
· Supervisory experience of a team of five (5) or more.
· The ability to prioritize and meet deadlines.
