  • Community Health Worker Supervisor-Butler Houses (DV)

    Title: Community Health Worker Supervisor

    Salary: Based on experience (35 hours per week)

    Basic Job Summary: 

    • CHW Supervisor will assist in scheduling of daily activities and will track and monitor program outcomes as well as produce and maintain detailed, accurate, and timely reports
    • CHW Supervisor will also ensure alignment of deliverables including the implementation of quality assurance, protocol and accountability of reporting processes to achieve contracted metrics and grant outcomes
    • CHW Supervisor will also support adoption of healthy behaviors by assessing community health needs and providing information on available resources, social supports, and navigation of resources
    • CHW Supervisor will also assists with the coordination and implementation of workshops/activities, client recruitment, referral services, follow-ups and site development

    Qualification:

    • Master’s Degree + 5 years of experience as a Program Manager/Coordinator/Supervisor at a nonprofit/CBO/government agency (NYCHA/DOH)
    • Excellent organizational and communication skills, with a high attention to detail
    • Proficiency with Microsoft programs, especially Excel, and other computer programs, applications, and shared networks
    • Commitment to working with community members, CBO staff and Government agencies including professionals in promoting health and diabetes or other chronic disease care
    • Knowledge of Bronx communities (Butler Houses is a plus)
    • Patience, persistence, adaptability and a sense of humor are all very helpful
